Permission to reproduce and use these fact sheets is not required.

Fact sheets must be used as published by The Children’s Hospital at Westmead, Sydney Children’s Hospital and Kaleidoscope Hunter Children’s Health Network with no alterations. It is permissible to add your organisations logo but such additions must not change the fact sheets in any way what so ever including the position of the hospital logos.

Fact sheets which have been modified in anyway may not carry the hospital logos or imply authority from the hospitals.

The following acknowledgement line needs to be added to the document where the fact sheet is being reproduced:

“The Insert name of your organisation acknowledges the co-operation of The Children’s Hospital at Westmead, Sydney Children’s Hospital and Kaleidoscope, Hunter Children’s Health Network in making this fact sheet available.”
